The professional gratification of handling bone deformity keeps us motivated: Dr. Ankit Chouhan

  • Your association with Narayan Seva Sansthan and experience?

I joined Narayan Seva Sansthan in October 2018. Previously, I was working with a private hospital in Udaipur for two and a half years as a Trauma & Deformity Correction Surgeon.

My interest in the field of Deformity Correction & Reconstructive surgeries started during my post-graduation (M.S.) days where I had the privilege of learning and training under immensely experienced surgeons and excellent teachers in the department of Orthopaedics at S.M.S. Medical College, Jaipur. Afterwards, I did one-year clinical fellowship in Ilizarov & Reconstructive surgeries at Center for Limb Lengthening and Reconstruction, Mumbai under Dr. Mangal Parihar where I did focused training in the field of Deformity Correction surgeries and learnt several methods like Software Assisted surgeries, Limb Lengthening Surgeries etc.

  • How have you brought a difference?

Narayan Seva Sansthan caters to the needs of a large number of patients with a variety of disabilities or deformities. Personally, I am inclined towards Bone Deformity Correction surgeries (Congenital & Acquired) using methods of Osteotomy and Fixation (Internal & External). With very enthusiastic support from the president of the organization, Mr. Prashant Agarwal, I have started application of Ilizarov technique at NSS. Ilizarov technique is a Russian origin method of Deformity Correction Surgery using metallic rings fixed to the bone with wires and screws. With this device a large number of complex deformities can be corrected; bones can be re-shaped to their normal structure, bone length can be increased and the patient can be restored to his/her functional status. I would like to mention that NSS might be the only one or one of the very few organizations in the country where this facility provided absolutely free of cost to the patients.

I have also started using Ortho-SUV system at NSS, which is a state-of-the-art Multi-Axis, Software Assisted Deformity Correction System applied over the Ilizarov assembly. This technique helps in correction of complex multi-dimensional bone deformities with precision.

With these new methods & techniques, we have widened the spectrum of corrective surgeries at NSS which now offers surgical correction & management of many complex and rare bone diseases like Congenital Pseudo-Arthrosis of Tibia, Tibial or Fibular Hemimelias, Radial Club Hand, congenital dislocation of hip, post-traumatic deformity, un-united fractures, short limbs etc.

  • How do you ensure that there is no compromise in quality?

Counselling, planning, surgical execution and follow-up are important to ensure quality of surgical work and better results. Before taking the patient to the operation theatre, all the necessary investigations like x-rays, MRI, CT scans or special blood tests are done. I use Software Simulation for planning corrective surgeries. This allows better understanding of the problem, helps pre-operative planning and saves precious surgical time.

Intra-operatively, use of C-Arm imagination is done to ensure accuracy at each step of the procedure. Strict Asepsis and standard OT protocols are followed to reduce risk of infections.

Bone fixation instruments like Ilizarov frame, plates and rods are available from a number of manufacturers with varying costs and standards. However, at NSS, we use only the standard devices and instruments. We provide free-of-cost surgeries but the quality of equipment, implants, medicines, prostheses etc. is at par with that provided at private hospitals.

During the follow-ups, the patient is thoroughly examined and counselled. Required medications is also provided. Physiotherapy and/or splints/calipers are also provided whenever required.

  • Your vision to carry this forward?

After working at NSS for six months, I still find myself overwhelmed by the volume and diversity of patients we receive on daily basis. NSS has a robust record of accomplishment for the last 25 years but I feel that there is still scope for improvement, which is what excites me to work with NSS.

All of us who are working for NSS share the dream of making it a Center of Excellence for Reconstructive & Rehabilitative surgeries and services. With continued support from our generous donors, we are consistently increasing our strength and improving the quality.

I am hopeful that very soon we may start Spine Deformity Correction surgeries, dedicated hand surgery unit, reconstructive plastic surgery, neurology clinics etc. In Orthopaedics, we have already started using the latest technologies in this field and we will continue upgrade.

NSS can also be a very good learning ground for the interested doctors and surgeons. I hope that we direct the dedicated efforts in conducting workshops, CMEs and fellowships for interested surgeons and associated professionals pertaining to this field. I have also started to collect and compile data for research and publication purpose. With the volume and variety of the patients at NSS, many research papers and projects can be undertaken here, augmenting our knowledge and reputation.

I wish to see NSS establish itself amongst the best medical institutions of our country.

  • Tell us about your team of doctors and what drives them?

Other senior orthopaedic surgeons working at NSS are Prof. (Retd.) Dr. A. S. Chundawat, Dr. O.D. Mathur and Dr. B.R. Shinde.  All three of them are highly experienced surgeons and have retired from prestigious government posts. Dr. Vaibhav Ghadoliya is the anesthetist at NSS.

The professional gratification of handling challenging cases everyday along with the personal satisfaction of being able to bring improvement in the quality of life of patients are the motivating aspects of working with NSS. I feel blessed to be associated with NSS where I am able to focus in my field of interest. Each day at NSS is filled with complex orthopaedics problems and challenging surgeries. Nevertheless, at the end of the day we all carry with us a sense of satisfaction of a job well done.
